What is Remote Patient Monitoring

RPM programs enable the collection of patient health data and vitals from the comfort of their own homes through the utilization of advanced technology and mobile health platforms that connect to the care team. The data can either be sent to the care team manually by the patient or it can be auto-transmitted to the care team without any patient involvement. RPM programs enable providers to continuously track and monitor patient data pertaining to their health, diet, activity, exercise, and environment, with the goal of improving a patient’s overall health and quality of life.

Benefits of RPM

Patient’s spend the majority of their time at home, not in the clinical setting. As such, the ability to continuously monitor a patient’s health outside of the typical care setting allows providers to create a care plan that is closely cohesive to a patient’s life. Implementing RPM programs to enable home-based care can produce a vast array of benefits to both your practice and your patients, including:

Decreased hospital admissions and readmissions
24/7 monitoring of patient’s health
Greater insight into a patient’s day-to-day health
Patients receive easier access to care, particularly beneficial for patients in rural areas
Reduced overall healthcare spending and transportation costs
Increased comfort for patients augments patient satisfaction and engagement
Optimize chronic care management capabilities and success rates
Increased communication and coordination among care team and patient
Reduce unnecessary in-person appointments

Coding

Services performed are charged based off of assigned codes, so it’s vital to continuously stay updated on current procedure codes, charging fees of each code, and the reimbursement rates set forth by insurance companies. Our team fervently stays updated on current insurance codes and regulations in order to successfully conserve the revenue management cycle and augment profits for your business.

Insurance Management

Simply put, we handle everything insurance-related. We’ll validate patient insurance and eligibility details, file the claims, ensure the prevention of denied claims, follow up with insurance companies, and promptly resolve existing denied claims.

Tracking the Process

We strive to make the billing process smooth and prompt by tracking the claim throughout its lifecycle and following-up with insurances to ensure steady progress. Additionally, we’ll take charge of filing patient paperwork, preparing refunds, mailing patient statements, managing any billing inquiry, and monitoring patient balances.

Chronic Care Management

RPM programs are an essential resource for patients with chronic conditions as healthcare providers can utilize RPM into their chronic care management plans to monitor the patient’s health between visits, provide immediate intervention in case of emergency, and determine necessary revisions to the care plan.

Post-Discharge Care

Effective transitional care management is vital to avoid unnecessary rehospitalizations, and RPM programs have been shown to be vital in monitoring a patient’s health post-discharge.

Seniors and People with Disabilities

The convenience, comfort, and freedom that RPM programs provide is especially beneficial to seniors and patients with disabilities who may struggle with transportation or who require additional attentive care.

Behavioral Health and Substance Abuse

Understanding the social and environmental challenges that someone faces helps healthcare providers better care for and manage the recovery of patients with behavioral health conditions, as the programs can help providers identify real-life challenges and be readily available in crisis times such as a relapse.

RPM 2020

The Centers for Medicare and Medicaid Services (CMS) had first stated that RPM codes could not be implemented “incident to” a physician’s service, but then later revised the rule and allowed “incident to” billing of RPM services by auxiliary personnel under direct supervision. An “incident to” services refers to a service done under the physician’s supervision, and billed to Medicare in the physician’s name. The revised rule allowing for direct supervision means that the auxiliary personnel must be in the same building as the physician at the same time, although not necessarily in the same room.
Effective January 1, 2020, CMS will allow RPM reimbursements for RPM services furnished “incident to” under general supervision, meaning that the auxiliary personnel and physician need not be in the same building at the same time, and can instead use telemedicine to exercise general supervision.
